Ask Dr. K

Iím a gay man who really enjoys being a bottom. After using a small dildo about two months ago, I started to have involuntary bowel movements for about two weeks. This really scared me. There was some blood the day after I used the dildo. I went to the doctor and he found internal and external anal warts and some hemorrhoids. He said the bowel movements were caused by an ulcer. The doctor recommended surgery for the warts. I still have some pain in my butt once in a while. This is really ruining my sex life and I want a second opinion. I am HIV+ and have a history of anal fissures.

Thereís no way for me to diagnose you without seeing you, but it does sound like you may have had some injury from using the dildo, or perhaps from some other recent sexual activity. The anus is a muscle and like other muscles in the body can tear, rip or shred with trauma. It can only stretch so far. Anal lesions, cuts, or ulcers, especially those deeper in the rectum, can change the dynamics of the large bowel and cause various symptoms, including rectal urgency (the feeling of the need to have a bowel movement), rectal bleeding, and/or painful bowel movements.

You might want to get a second opinion, since it sounds like you feel that you haven't gotten enough information from your medical provider. Once you decide on the next step and are comfortable again, there are some great books you can read to help you with anal play. Anal Pleasure & Health by Jack Morin is a great resource.

To your health,
Dr. K
